How to Apply

Submit a completed application and transcript by March 5, 2010. A completed
application includes all of the following 4 items:

	Application Part I
	Application Part II
	Two letters of recommendation
	Official Grade Transcripts

Application Part I

1. Choose your method of application. Send either a paper or an online application. * Do not send both.

We strongly encourage you to apply online.

Once you complete Part I of the online application, you will receive a notice to return to this page to complete Part II of the application.

If you are unable to access or view the application, you can obtain the application by writing to: Whitney Sweeney (email: sweeney@biostat.wisc.edu) at the address listed below.

Send Application Part II along with transcripts to the address below.

*NOTE*- If you choose not to submit an online version of Application Part I, remember to mail a completed paper copy of Application Part I along with Application Part II.

Letters of Recommendation

3. Submit 2 letters of recommendation to the address below. Letters may either be sent directly, or submitted in sealed, signed envelopes along with Application Part II.

Official Grade Transcripts

4. Please send official transcripts to the address below. We need transcripts to make a decision.
